Notice of Privacy

This notice describes how medical information about you may be used and disclosed and how you can obtain access to this information. If you have any questions about the Notice, please contact our Privacy Officer, Georgene Saylor.

Treatment: We will use and disclose your protected health information to provide, coordinate, or manage your healthcare. For example, your information will be shared with Emergency Room staff to assist with your treatment and care.

Payment: Your protected health information will be used to obtain payment for your health care services. For example, to obtain approval for a ambulance transport may require your information to be disclosed to your health insurance company.

Healthcare Operations: We may use or disclose your protected health information in order to support our business activities. For example, we may disclose your information to a student that will be helping with your care.

Facility Directory: We will use and disclose in our locked files your name, location, your condition and any other information pertaining to your medical information.

You have the right to inspect and receive a copy of your protected health information. You have the right to request a restriction of your protected health information. You have the right to request to receive confidential communications from us, by alternative means or location. You have the right to amend your protected health information. You have the right to receive an accounting of certain disclosures we have made, if any, of your protected health information. You have the right to file a complaint if you believe your privacy rights have been violated by us.
